If you need to reschedule, please notify us at least 24 hours before your appointment, otherwise you will be charged a cancellation fee of $65.

If you wear glasses, please be sure to bring all the glasses you currently use to your appointment.

If you are a contact lens wearer, please come in wearing your current contacts. Be sure to know the brand, prescription powers for each eye or bring the box that they came in.

Please bring your medical insurance cards and driver's license. We will be scanning these into your records.

Optomap Retinal Imaging is a part of your comprehensive eye exam and will reduce or eliminate the need for a dilated eye exam. For those with Medicare, the fee for this procedure is $39.
